In 1979, the teenage Mfazi joined the Congress of South African Students (COSAS), immersing himself in student resistance to apartheid. As police persecution intensified, he made the perilous journey into exile — crossing borders to join uMkhonto weSizwe (MK), the armed wing of the African National Congress, in Angola. He rose from combatant to commander, earning a reputation as a soldier of extraordinary courage and composure. He fought in the brutal bush wars against Jonas Savimbi’s UNITA forces — the “Savimbi bandits” backed by apartheid South Africa and the CIA — and in those blood-soaked Angolan trenches, he stood shoulder to shoulder with none other than Chris Hani, sharing the same battle lines in one of the liberation movement’s most ferocious theatres of war. Those who trained under him in exile described him as fearless, disciplined, and principled — a commander who never asked his troops to go where he himself would not. He remained in exile until 1991, when he returned to South Africa ahead of the democratic transition, carrying the scars and the steel of a true freedom fighter.

When the new democratic South Africa was born after the 1994 elections, the MK cadres were amalgamated into the South African Police Service. Mfazi entered the SAPS at the rank of Warrant Officer, assigned to the Protective Services division — safeguarding the nation’s most senior leadership. He was based in the Eastern Cape, the heartland he had always called home. It was in this role that one of the most remarkable chapters of his career quietly unfolded: Nelson Mandela, President of the Republic of South Africa, would personally call Mfazi directly whenever he was travelling home to Qunu in the Eastern Cape. The President trusted him. That personal bond and direct line of communication with Mandela was a source of immense pride — but it was not without cost. The sight of a Warrant Officer receiving personal calls from the President angered his superiors in the police at the time, who viewed it as a threat to the chain of command. It was an early signal of what would follow throughout his career: a man whose ability and integrity consistently outgrew the structures that surrounded him.

Within the SAPS, Mfazi rose to serve as Provincial Head of Crime Intelligence in the Eastern Cape — a post that would become the defining chapter of his operational career. He also served as Deputy Provincial Commissioner of the Western Cape before being called back to his home province, the Eastern Cape, to confront a worsening crisis: the dramatic rise of cash in transit heists that were terrorising the country and making South Africa’s roads a war zone for cash delivery crews. It was here that Mfazi’s intelligence genius was most fully expressed. Under his command, his teams became the first in South African policing history to successfully infiltrate the cash in transit gangs using a deep cover agent — a covert operator embedded so deeply inside the criminal networks that the gangs never saw the net closing around them. The intelligence gathered was painstaking, meticulous, and ruthlessly precise. Then in 2008, at the height of the cash in transit heist epidemic, General Mfazi coordinated what would go down in SAPS history as the largest single takedown of cash in transit robbers ever executed in South Africa — a massive, simultaneous operation that dismantled the network from the inside out. It was a masterclass in intelligence-led policing that left the country stunned and the underworld reeling.

The results spoke for themselves. His decade of exceptional intelligence leadership, the groundbreaking deep cover operations, and the historic 2008 takedown cemented his standing as one of the finest operational minds in the SAPS. He was eventually promoted to SAPS Head Office in Pretoria, ascending to the rank of Deputy National Commissioner — one of only two officers in the entire service at that level. He first served as Deputy National Commissioner of Management Advisory Services from 2018, then in 2019 briefly stepped up as Acting Provincial Commissioner for the Western Cape. In March 2020, he was appointed Deputy National Commissioner: Crime Detection, overseeing Forensic Services and the most serious and complex investigations in the country. He had travelled from a Warrant Officer in Protective Services to the second-highest rank in South African policing. The boy from Mdantsane had reached the very top.

From March 2020 until his death, Mfazi led Crime Detection as Deputy National Commissioner — overseeing Forensic Services and what would become the most explosive cluster of investigations in recent South African policing history. He did not conduct one dangerous investigation. He conducted four. The first was the R1.6 Billion PPE Corruption probe. At the onset of Covid-19, the SAPS procurement system was flooded with fraudulent PPE tenders — billions rand siphoned from public funds through inflated contracts for gloves, masks, and sanitisers. Mfazi’s division identified a network of corruption that allegedly implicated the SAPS Secret Services Account and reached the very top of the police hierarchy. He refused to bury the findings. The second was the R6.1 Billion FDA Corruption investigation. His division began tracking a massive fraud scheme involving the misappropriation of state funds through irregular financial arrangements — an investigation that pointed squarely at politically connected individuals and threatened to destabilise powerful structures within and beyond the police service. The scale of this investigation — R6.1 billion — made it one of the most consequential corruption probes ever initiated within the SAPS. The third was the Phala Phala probe. Mfazi’s Crime Detection division became embroiled in the early intelligence and forensic threads of what would later become known as the Phala Phala scandal — allegations of undisclosed foreign currency hidden at President Ramaphosa’s Phala Phala wildlife farm in Limpopo, and a subsequent cover-up involving senior security and intelligence officials. His access to the intelligence architecture of the state made him uniquely positioned — and uniquely dangerous to those with something to hide. The fourth was the broader SAPS Internal Corruption Investigations — a systematic examination of corruption within the police service itself, including the abuse of the Secret Services Account, irregular appointments, and the capture of senior SAPS structures by criminal and political interests. This was the investigation that struck closest to home for those in command. Those closest to him say he understood the danger. His family knew the investigation files he carried. He pressed on regardless. By early 2021, colleagues described him as a man who had made peace with the risk — but was determined to see it through. It was the last great battle of a 39-year career. And it made him a marked man.

Lt General Mfazi died on 8 July 2021. The official SAPS statement attributed his death to Covid-19 complications. But his family immediately raised alarm — describing chilling symptoms in his final days: vomiting blood, and bleeding from his ears, mouth, and eyes. Just 10 days after his burial, the family demanded an exhumation and independent toxicology tests. The results were devastating: liquid casting resin — a lethal industrial poison — was found in his remains. His death is now the subject of an active murder investigation.

What happened after his death deepened the suspicion. According to SAPS sources and parliamentary testimony, senior police officials entered his home immediately after his death — while his body was still on the premises — and removed sensitive dockets, investigation files, and his laptop. A murder docket was eventually opened at Cambridge Police Station. But in February 2026, a former Crime Intelligence officer testified before Parliament that this investigation is being deliberately suppressed by SAPS Head Office. The call for justice has never been louder.

Behind the general was a devoted family man. He is survived by his beloved wife Nomisumzi, whom he affectionately called “Malindi”, and their two sons Gugulethu and Lutho. Colleagues remembered him as a “gentleman” and mentor who was “fearless in espousing his viewpoints” and held his family as his highest priority. His 39-year service — from a COSAS student activist in Mdantsane to the Deputy National Commissioner of South Africa’s police service — is a story of extraordinary courage, sacrifice, and unwavering service to the people of South Africa.
